Patient forms will be provided to you at the time of scheduling or upon request. You may also call our office to receive forms in advance of your visit.
New Patients: Please arrive 15 minutes early if you have not completed your forms in advance. Bring a photo ID, your insurance card, and a list of current medications to every appointment. To request forms in advance, call us at 512.528.1144.
Your Paperwork at a Glance
New Patient Registration
Basic demographic information, emergency contacts, and primary care provider information. Required for all new patients before your first visit.
Medical History Form
Comprehensive eye and medical history including current medications, allergies, prior surgeries, and family eye health history.
Insurance Information
Your insurance card details, policy holder information, and authorization for billing your plan for covered services.
HIPAA Authorization
Authorizes Hill Country Eye Center to use and disclose your protected health information as required by federal law.
Forms Available Upon Scheduling
When you schedule an appointment, our team will provide you with the necessary forms. If you would like to receive them in advance to complete before your visit, simply call us at 512.528.1144 and we will send them to you.
Established patients can also message our care team and request forms or document updates through our secure patient portal. Ask our front desk for your portal activation link.
