Refractive Lens Exchange
RLE replaces the eye’s natural lens before a cataract can form — correcting severe refractive errors and providing clear vision at all distances. It’s the ideal alternative for patients who are not good candidates for LASIK or PRK.
Ideal Candidates for RLE
RLE is generally recommended for patients with severe refractive errors — high myopia, hyperopia, or presbyopia — for whom laser vision correction (LASIK/PRK) is not ideal. It may also be recommended for patients with early-stage cataracts or those at high risk of developing cataracts.
✓ Good Candidates
- Over age 40 with stable prescription
- High nearsightedness (myopia)
- High farsightedness (hyperopia)
- Presbyopia (difficulty with near vision)
- Thin corneas or dry eyes (poor LASIK candidate)
- Early-stage cataracts or high cataract risk
Typically Not Recommended If…
- Prescription is still changing
- Under 40 with strong accommodation
- Mild refractive error (LASIK may be better)
- History of certain retinal conditions
What to Expect: Before, During & After
Pre-Operative
- →Comprehensive eye exam to confirm candidacy
- →Discussion of IOL types available
- →Stop wearing contact lenses to allow corneas to normalize
- →Arrange a driver for the day of surgery
The Procedure
- →Outpatient — no overnight hospital stay
- →Local anesthetic to numb the eye
- →Natural lens removed with ultrasound or laser
- →Artificial IOL implanted through a small incision
Recovery
- →Some discomfort and light sensitivity — resolves quickly
- →Eye drops prescribed for infection and inflammation
- →Avoid strenuous activity and rubbing eyes for several days
- →Multiple follow-up appointments to monitor healing
Key benefit over LASIK: Because RLE removes the natural lens entirely, you can never develop a cataract in that eye in the future. It’s a permanent, one-time correction that eliminates the need for cataract surgery later in life.
